The Aetna Vision Preferred plan covers routine vision services such as routine eye examinations, eyeglass frames, eyeglass lenses, lens options and contact lenses. Additionally, you'll receive fixed member pricing on premium lens options such as premium progressive lenses, anti-reflective coatings, and photochromic lenses that darken in sunlight and are clear indoors.

If you choose the standard option plan,

You will pay the copay listed below; the plan pays the rest:

$0 copay for your routine eye exam
$10 copay for lenses (standard plastic single, bifocal, trifocal and lenticular lenses)
$0 copay for these lens options; standard plastic scratch coating and polycarbonate lenses
$0 copay for medically necessary contact lenses

The plan will pay the allowance listed below; you will pay the balance:

$120 allowance towards the purchase of conventional or disposable contact lenses
$120 allowance towards the purchase of frames (even frames for prescription sunglasses)

If you choose the standard option plan, you will get an exam and lenses (contacts or eyeglass lenses) every calendar year and frames every other calendar year.

Discounts on services and materials have no frequency on how often you can use them

No. You can visit any licensed vision provider. Aetna does have a network with over 77,000 vision providers in independent offices and retail locations. In fact, we offer one of the largest national networks with providers in all 50 states including Puerto Rico and the U.S. Virgin Islands.

Your out of pocket costs may be significantly lower if you use an in-network provider from our national network. Keep in mind that our network includes a wide range of providers who can meet the needs of the fashion focused, value conscious, and everything in between. If you choose to use a vision provider who is not in the network, you may submit a claim and be reimbursed up to the maximum out of network amount allowed by the plan. You will need to go directly to www.contactsdirect.com. From there, all you need is your contact lens prescription and vision insurance information.

You'll begin by registering a new account.

  1. Click on register in the top navigation.
  2. Fill out registration form.
  3. Check the box to apply your vision insurance.
  4. ContactsDirect will check our database to validate your insurance information and apply your vision coverage at checkout, including any additional discounts you may be eligible for.

You must have a valid prescription in order to use this program, which will be verified with your doctor before shipping. Upon the prescription release, the requested lenses will be conveniently and immediately shipped to you. Most orders will be delivered within 48 hours and 90% of orders ship within 24 hours. Overnight and rush delivery are available for an additional charge

You will need to go directly to www.glasses.com. From there, all you need is your eyeglass prescription and vision insurance information.

You'll begin by registering a new account.

  1. Select Vision Insurance from the top navigation
  2. Complete the registration form (name, DOB, zip code or member ID and plan number)
  3. Sign in and start browsing for the perfect eyeglasses

Shop by collection, by brand, 3d virtual try-on app or in-home try on. Once you've found the frame to fit your style, select your lenses & checkout (even progressive and multifocals are available). Provide a valid prescription (not sure of your prescription? Give your doctors name and phone number and they will contact your doc for you). Once confirmed your order is quality check & shipped, for free. If your glasses aren't exactly what you want return them for free within 30 days for a full refund. You can even take your new glasses to LensCrafters for free frame adjustments.

Ophthalmologists, optometrists, and opticians are included in our network to provide access to a full range of eye care services to you.

Yes. You can ask your eye care professional to complete this Provider Nomination form and return as instructed on the form. Our provider relations team will send a recruitment packet to your provider. However, it is not a guarantee that a provider will join our network.

No. You are free to choose any licensed vision care provider. You can even select one provider for your eye exam, and receive your materials (frames, lenses, contacts) from a different provider. There is no need to tell us who you intend to visit and no pre-authorization is required.

Our Docfind provider directory is updated nightly so the most current information is always displayed.

No. The plan covers routine vision services only. Check your medical plan to see if you have coverage for non-routine vision services.

No. There are no deductibles included with our Aetna Vision Preferred plan.

Using your vision benefit at a network provider is easy.

  1. Locate a provider that services your plan by using our provider locator, by calling our Customer Service Center or use the Aetna Vision Preferred mobile app on-the-go (available in the App Store)
  2. Schedule an appointment.
  3. Identify yourself as an Aetna Vision Preferred member. Show your ID card or provide your name and date of birth at the time of your appointment.
The provider's staff will do the rest! You will only pay for any copays indicated, as well as any applicable amounts over the allowances. Your provider will supply you with these amounts.

The process varies slightly if you decide to use a non-network provider.

Request an Out-of-Network Claim Form: You can call our Customer Service Center or go to our website to request/print an out-of-network claim form before receiving services.

Receive Services and Submit Claim Form: After receiving services, request an itemized paid receipt and submit the receipt along with the claim form – either mail, fax or submit online, it's simple!

Reimbursement Check mailed to Subscriber: Once the claim is processed, a reimbursement check will be mailed directly to you.

Once you are enrolled, we will provide extensive live-agent support Monday through Saturday 7:30 a.m. – 11:00 p.m. ET and Sunday 11:00 a.m. – 8:00 p.m. ET. And with our state-of-the-art IVR system and self-service website, we are able to serve you around the clock, offering automated features 24 hours a day.

No. Coverage is not provided for replacement of lost, stolen or broken materials. Our network providers will honor all manufacturer product warranties and they may offer their own breakage protection programs at an additional cost. Be sure to check with your provider.

Yes. You will receive a welcome packet after you enroll. It will include your member ID card, basic plan information and a list of providers near you. Once active, you'll also have the ability to print a card online.

Once you are enrolled, you'll be able to access your personalized, secure member portal on Aetna Navigator® or www.aetnavison.com. You'll find lots of great features, such as;

Hi-index lenses are considered a non-covered service under both the standard and high plan offerings. If you use a participating provider, you will receive a 20% discount off the retail price. There is no reimbursement when an out of network provider is used.

Safety glasses are standardly not covered but you will receive a 20% discount off the retail price at in-network provider locations that have them in stock.

Standard contact fitting is the applications of clear, soft, spherical, daily-wear contact lenses for single-vision prescriptions. You will pay $55 when you visit a participating provider. There is no reimbursement when an out of network provider is used. Premium contact lens fitting is a more complex applications, including (but not limited to) toric (astigmatism .62D or higher in the contact lens), multifocal/monovision, post-surgical and gas permeable lenses. Includes extended/overnight wear for any prescription. You will receive a 10% discount off the retail price when you visit a participating provider. There is no reimbursement when an out of network provider is used.
