About the plan

FEDVIP is administered by the U.S. Office of Personnel Management. It is a voluntary, enrollee-pay-all dental and vision program. It’s available to:
- Federal employees and annuitants
- Retired uniformed service members
- Families of active duty service members and their eligible family members

The Aetna Vision Preferred plan covers vision services such as routine eye examinations, eyeglass frames, eyeglass lenses, lens options (like scratch resistant coating) and contact lenses. You'll also get fixed member pricing on premium lens options such as premium progressive lenses, anti-reflective coatings, and photochromic lenses that darken in sunlight and are clear indoors.

Your benefits include an exam, and an allowance to purchase frames and lenses (contacts or eyeglass lenses) every calendar year.
There’s no frequency limit for discounts on additional pairs of glasses and conventional contact lenses.

Yes. Eye exams include dilation as necessary at no additional cost.

No. The plan covers routine vision services. Check your medical plan to see if you have coverage for non-routine vision services.

No. Hi-index lenses are considered a non-covered service under both the standard and high plan options. If you use a participating provider, you will receive a 20% discount off the retail price. There is no reimbursement when an out-of-network provider is used.

No. Safety glasses are not covered, but you will receive a 20% discount off the retail price at in-network provider locations that have them in stock. You can find vision providers by using our provider search. Select “Filter,” then “Products,” then “Safety.”

No. The replacement of lost, stolen or broken materials isn’t covered. Our network providers will honor all manufacturer product warranties. They may offer their own breakage protection programs at an additional cost. Be sure to check with your provider.

Standard contact fitting is the application of clear, soft, spherical, daily-wear contact lenses for single-vision prescriptions. You’ll pay $40 when you visit an in-network provider. There is no reimbursement when you use an out-of-network provider.
Premium contact lens fitting is conducted for more complex applications, including (but not limited to) toric (astigmatism .62D or higher in the contact lens), multifocal/monovision, post-surgical and gas permeable lenses. This includes extended/overnight wear for any prescription. You’ll get a 10% discount off the retail price when you visit an in-network provider. There’s no reimbursement when you use an out-of-network provider.

Members who live overseas can submit a claim for reimbursement up to the maximum out-of-network amount allowed by the plan. Or they can shop online at any of our in-network providers and have their glasses or contacts shipped directly to their location.
Doctors and network

No. You can visit any licensed vision provider. But remember, visits to providers in the Aetna network can cost you less. With over 140,000 participating vision providers in independent offices and retail locations, it’s easy to find one near you. In fact, we offer one of the largest national networks, with providers in all 50 states, Puerto Rico and the U.S. Virgin Islands.
If you choose to use a vision provider who is not in the network, you may submit a claim. You could be reimbursed up to the maximum out-of-network amount allowed by the plan.

Ophthalmologists, optometrists, opticians and eyewear dispensaries are part of our network. They’ll give you access to a wide spectrum of eye care services.

Costco is an allied provider. Allied providers are not in the network. But they will handle the claim submission process for you and you'll only pay for anything above your maximum benefit.

No. You can visit any licensed vision provider. You can even select one provider for your eye exam, and receive your frames, lenses or contacts from a different provider. You don’t need to tell us who you intend to visit, and no preauthorization is required.

You can submit your out-of-network claim form online or by mail. After receiving services, ask for an itemized paid receipt. Submit the receipt along with the claim form — either by mail, fax or online. Allied providers, like Costco, will submit your out-of-network claim for you.
You can use the Vision Claim Form to submit your itemized paid receipts.

Request an out-of-network claim form: Visit AetnaFeds.com/resources to obtain the claim form. Or call Member Services for a form.Once the claim is processed, a reimbursement check will be mailed directly to you.
What will I pay?

No. There are no deductibles included with our Aetna Vision Preferred plans.

If you choose the standard option plan:
You’ll pay the copays listed below. The plan pays the rest:- $0 for your routine eye exam
- $10 copay for lenses (standard plastic single, bifocal, trifocal and lenticular lenses)
- $75 for standard progressive lenses
- $95–$120 for most premium progressive lenses
- $0 for standard plastic scratch coating and polycarbonate lenses
- $0 for medically necessary contact lenses
- $150 allowance toward the purchase of conventional or disposable contact lenses
- $160 allowance toward the purchase of frames (even frames for prescription sunglasses)
If you choose the standard option plan, you’ll get an exam, lenses (contacts or eyeglass lenses) and frames every calendar year.
When you stay in network, you'll also receive the following discounts:

Enroll with BENEFEDS during Open Season, November 11 through December 9, 2024, or after a qualifying life event that permits enrollment outside of Open Season. New employees will have 60 days from when they start working to enroll.

Coverage and 2025 benefit changes for members and annuitants begins on January 1, 2025.
If you enroll as a newly hired federal employee, you have 60 days to enroll in FEDVIP. Your enrollment will become effective the beginning of the pay period after your enrollment is received.

Using your vision benefit at a network provider is easy.
Find a provider that accepts your plan by using our provider search. You can also call us or use the Aetna Vision Preferred mobile app on the go.
Schedule an appointment.
Identify yourself as an Aetna Vision Preferred member. Show your ID card or provide your name and date of birth at the time of your appointment. The provider's staff will do the rest! You will pay for any copays indicated, as well as any applicable amounts over the allowances. Your provider will supply you with these amounts.
The process varies slightly if you decide to use an out-of-network provider.

Yes. New members will get a welcome packet after you enroll. Once active, you can also print a card from your member website.
